GLIN is seeking a Nurse (RN) Care Manager to coordinate the care and provide needed assistance during the immediate post-inpatient/ rehab period of time for patients, their family and/or caregiver across the continuum. Overcoming health care barriers and facilitating timely referrals for appointments to appropriate specialists. Serves as a resource to patients, physicians and others across the continuum of services. Outreaching patients telephonically.Duties and Responsibilities (including but not limited to)
To provide education related to diagnosis, treatment, support services and community resources based on disease
To facilitate timely coordination of appointments, tests and procedures communicating any concerns to the appropriate physician/s.
To provide a comprehensive assessment of the patient, family and / or caregivers needs at a medical visit across the continuum, including assessing barriers to care and referring to appropriate disciplines.
To educate the patient, family and/or caregiver on signs and symptoms of complications regarding their specific diagnosis and when to call their PMD.

Qualifications or Education, Training, and Experience:
Education
Associate of Applied Science in Nursing required
Bachelor of Science in Nursing preferred
Work Experience
Two years' experience in Community based/Primary Care Nursing required
Licenses and Certifications
Registered Nurse License required upon hire
NYS Drivers License required upon hire
Case Management Certification preferred
