  • Implementing person-centered support plans and daily routines tailored to each individual's goals and preferences.
  • Maintaining a safe, clean, and supportive home environment.
  • Assisting with personal care activities, including grooming, hygiene, dressing, toileting, and medication management.
  • Supporting household tasks such as laundry, meal preparation, grocery shopping, and general cleaning.
  • Providing transportation to medical appointments, recreational outings, community integration activities, and family visits.
  • Promoting social interaction, independence, and self-advocacy.
  • Accompanying clients to urgent care, emergency rooms, and documenting all health-related outcomes.
  • Writing detailed and timely daily progress notes and reports.
  • Participating in developing and implementing Individual Services Plans (ISP) and Behavior Services Plans (BSP)
  • Attending required staff meetings, training sessions (minimum 12 hours annually), and ongoing supervision.
  • Supporting goal achievement through teaching daily living skills and reinforcing positive behaviors.
Qualifications
  • Experience: Minimum of 1 year of experience providing direct care or supervision to individuals with developmental and/or intellectual disabilities.
  • Clearances: Must pass DOJ/FBI background check (Live Scan clearance required before start date).
  • Education: High school diploma or GED required.
  • Certifications: Current CPR and First Aid certification required.
  • Driver Requirements: Valid California driver's license, proof of current auto insurance, DMV record, and reliable transportation.
  • Preferred Skills: Experience with behavior modification programs and working with individuals with various medical conditions is highly desirable.
  • Soft Skills: Strong commitment to compassionate care, patience, flexibility, reliability, and integrity.
  • Communication: Excellent communication and interpersonal skills.
  • Safety: Ability to adhere to all health and safety guidelines
