- Implementing person-centered support plans and daily routines tailored to each individual's goals and preferences.
- Maintaining a safe, clean, and supportive home environment.
- Assisting with personal care activities, including grooming, hygiene, dressing, toileting, and medication management.
- Supporting household tasks such as laundry, meal preparation, grocery shopping, and general cleaning.
- Providing transportation to medical appointments, recreational outings, community integration activities, and family visits.
- Promoting social interaction, independence, and self-advocacy.
- Accompanying clients to urgent care, emergency rooms, and documenting all health-related outcomes.
- Writing detailed and timely daily progress notes and reports.
- Participating in developing and implementing Individual Services Plans (ISP) and Behavior Services Plans (BSP)
- Attending required staff meetings, training sessions (minimum 12 hours annually), and ongoing supervision.
- Supporting goal achievement through teaching daily living skills and reinforcing positive behaviors.
- Experience: Minimum of 1 year of experience providing direct care or supervision to individuals with developmental and/or intellectual disabilities.
- Clearances: Must pass DOJ/FBI background check (Live Scan clearance required before start date).
- Education: High school diploma or GED required.
- Certifications: Current CPR and First Aid certification required.
- Driver Requirements: Valid California driver's license, proof of current auto insurance, DMV record, and reliable transportation.
- Preferred Skills: Experience with behavior modification programs and working with individuals with various medical conditions is highly desirable.
