ESSENTIAL DUTIES & RESPONSIBILITIES
- Oversight and management of Certified Nursing Assistants (CNAs), including recruitment, scheduling, performance management and annual evaluations. Three CNAs are on duty at all times.
- Supervision of LVN including recruitment, scheduling, performance management and annual evaluations. Share best practices to ensure smooth operations and provide support as needed. Serve as RN Case Manager to ensure data systems and provision of care satisfy Catholic Charities Best Practices Standard of Care.
- Participate in resident intake process as a member of the clinical intake teams in conjunction with the SF. Coordinated Entry System.
- Discharge planning and associated implementation as needed.
- Manage care of residents and ensure coordinated responsibilities from other staff are executed. Be available for instruction and consultation.
- Communicate with health service providers on an ongoing basis to report health status changes, advocate for medical intervention, and assure coordinated, quality care for all residents as needed.
- Manage process for the documentation and safe administration of all residents’ medication as outlined in the Medication Policies and Procedures. This includes quality checks upon medication delivery from the pharmacy (Medicine On Time [MOT] system), maintaining current medication profiles, assessing for adverse effects of medications, coordinating reorders and medication changes, and monitoring the use of controlled substances. Troubleshoot and resolve any discrepancies.
- Manage monthly maintenance of MOTs. Log in all new arrivals of medications.
- Oversight and management of dispensing medications to residents as per policy (Routine, PRN, Narcotics).
- Develop and implement Individual Service Plan and provide input for CNA Care Plans and update as required.
- Monthly documentation and upkeep of ongoing client progress notes.
- Complete quarterly Nursing Individual Service Plans (ISPs).
- Respond to and follow up on medical incident reports in compliance with licensing standards.
- Complete monthly antiretroviral (ARV) compliance report.
- Manage emergency medical situations.
- Manage TB Surveillance and health related Quality Assurance activities.
- Manage administration of annual flu shots. This includes coordination with DPH for vaccine procurement.
- When scheduled for on-call rotations, answer calls when required.
- Part time on call duty for Leland House Nursing.
- Part time administrator on call duty.
- Attendance at weekly team oriented clinical meetings.
- Attend and participate in case conferences and staff meetings when required.

- Experience in assessment and strong crisis intervention skills recommended, including matters of substance abuse and mental health issues.
- Good working knowledge of Harm Reduction principles and practice.
- California license in good standing as a Registered Nurse.
- Bachelors Degree preferred.
- Experience with case management of HIV infected adults, knowledge of HIV disease, substance abuse, and mental health issues.
- Ability to make sound decisions and anticipate situations before occurrence.
- Ability to design systems to track data and monitor program process.
- Ability to work with complex objectives.
- Ability to communicate clearly and comfortably, both verbally and in writing.
- Ability to think analytically, supervise and direct others, and develop and foster teamwork.
- Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in cross-cultural situations.
- Is responsible for accurate and timely data entry that aids Performance and Quality Improvement (PQI) reporting and case record reviews.
- Is responsible for accurate and timely submission of case records if needed.
- Frequent bending, standing, stooping, kneeling, reaching, twisting, and walking.
- Intermittent lifting, pushing, and pulling.
- Frequent repetitive motions: Making substantial movements (motions) of the wrists, hands, and/or fingers.
- Medium work: Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
- The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; and extensive reading.
- Driving is required for this position.
- Contact with clients who may have behavioral and psychiatric problems. Behaviors may include shouting, use of profanity, inappropriate behavioral choices.
- Noise level in work environment is usually moderate and consistent with a normal office setting.
- Occasionally exposed to perfume or scents in personal care products used by clients.
- Exposure to possessions and/or artwork of residential clients due to room entry that may be offensive to some.
- The work environment is within the care facility of 45 residents and staff are here to support them.
