ShelterCare's Medical Respite (SMR) program of Eugene, OR is seeking to hire a full-time Community Support Specialist. This position works directly with individuals experiencing homelessness and have recently been discharged from the hospital after an acute medical episode, yet still require limited care. During their 30-day, post-hospital recuperation, individuals can focus on their health needs while our Community Support Specialist help them create sustainable connections to community resources and medical providers.See attached document for full position description.The Community Support Specialist is a non-exempt hourly position. The starting wage for this position is $20.13 to $20.48 per hour; depending on a number of factors.This position is considered essential and will be required to work holidays as scheduled. If it's a ShelterCare paid holiday, you will receive holiday pay plus pay for your time worked. The schedule for this position is Tuesday through Saturday.What you bring to ShelterCare:Required
  • 3 years in healthcare or social services

  • Bachelor's degree in healthcare, social services or a related field
Essential SkillsEffective Communication; Negotiation and Collaboration; Scheduling and Time Management; Leadership; Risk Management; Critical Thinking and Problem-Solving; Use of technology for email spreadsheets, documents, etc.PreferredPrior electronic health records experience is strongly preferred; Experience working with individuals diagnosed with severe and persistent mental illness.ABOUT SHELTERCAREShelterCare is a social services agency that is directed by a board of community volunteers.
