
Clinical Nurse Coordinator - Outpatient Oncology

- In collaboration with the providers, completes pre and post procedural calls with patients providing them instructions and their caregivers.
- In collaboration with the practice manager, assists the staff with pre-visit planning to complete tasks associated with closing gaps in care and meeting quality measures.
- Serves as a clinical resource/educator to facilitate the development of the team in collaboration with the providers, practice manager, clinical leads, and care manager.
- Reviews quality measures with the Director of the Medical Group, Program Consultant, and Director of Operations.
- Reviews quality measures with the providers and office staff and collaboratively works to develop initiatives to improve or maintain those measures.
- In collaboration with the practice manager, assists with staff training and the creation of standard work.
- Ensures consistent application of medical group rules and regulations.
- Participates in the development and implementation of practice guidelines, procedures, and standards. Including documentation and follow-up.
- Supports the team with care coordination.
- Performs related duties as assigned.
- 1 year of care coordination and/or understanding of Oncology practice dynamics is Required.
- Current Maryland Nursing License is Required.
- BLS is required.
- Bachelor’s degree (or in progress) is preferred.
- Care Management Certification is preferred.
- Knowledge of quality-based reimbursement and utilization management.
- Ability to make sound, independent judgements and act professionally under pressure .
- Ability to apply creative problem-solving skills.

- Pay Range: $40.61- $60.96
