- Responsible for coordinating and recommending appropriate services for residents when requested and as necessary to assist them in maintaining a self-reliant lifestyle.
- Responsible for maintaining organized files on each resident in the American Association of Service Coordination online file program and physical paper files following the GENACROSS standardized file procedures.
- Responsible for maintaining appropriate documentation on resident services and resident issues, including progress notes, following the GENACROSS documentation guidelines.
- Responsible for scheduling and completing resident apartment visits with each resident on an annual basis to update files and assessments.
- Responsible for completing appropriate and mandatory training, such as training through the Ohio Senior Health Insurance Information Program and ethics training, to maintain the highest standards in the GENACROSS Lutheran Services.
- Responsible for immediately reporting any resident issues or incident reports to Senior Level Service Coordinator and Director of Service Coordination.
- Responsible for reporting suspected abuse, neglect, or exploitation by a resident or others to Adult Protective Services (when possible, consult with the Senior Level Service Coordinator and Director of Service Coordination prior to making such APS reports).
- Responsible for completing and submitting monthly reports to Senior Level Service Coordinator by the deadlines determined.
- Responsible for assisting in the development of partnerships with local Member Congregations, Lutheran Parishes, and other Denomination Parishes, respectfully.
- Complete reports and files related to PASSPORT Certification as instructed.
- Act as a Transition Coordinator for any referrals assigned by the Senior Level Service Coordinator through the Home Choice Program.
- Assures the highest quality, efficient and effective service delivery.
- Bachelor's degree in social work or related field preferred. LSW/LISW preferred. Equivalent in related experience may suffice.
- Valid driver's license required.
- Requires strong oral and written communication skills.
- Understands directions, communicates and responds to inquiries promptly; has excellent human relation skills
