In this role, the dietitian will serve as a Diabetes Educator, providing individualized Medical Nutrition Therapy (MNT), Diabetes Self-Management Training (DSMT), and insulin pump management for patients with diabetes and pre-diabetes. Operating under minimal supervision, this position involves delivering personalized nutrition care to both individuals and groups for a variety of nutrition-related concerns, including chronic disease management, prevention, and overall well-being. The dietitian is responsible for assessing, planning, implementing, monitoring, evaluating, discontinuing, and documenting nutrition care plans while adhering to clinic-specific protocols and workflows. The successful candidate will demonstrate professional competency and actively participate in compliance and quality improvement initiatives to enhance the standard of nutrition and diabetes care across the organization.EXPERIENCE DESCRIPTION:A minimum of two years of experience with direct patient care is preferred.EDUCATION DESCRIPTION:Bachelors or Masters degree from an accredited Clinical Dietetics and Nutrition program is required. Completion of an ACEND approved internship or coordinated undergraduate program is required.LICENSURE DESCRIPTION:Current Registered Dietitian Nutritionist (RDN) is required or must be registration eligible. Current RDN must also be a State of Wisconsin Certified Dietitian or show proof of application for certification within 60 days of hire or transfer.
