  • Perform in-home assessments and develop individualized treatment plans based on patient needs.
  • Provide one-on-one therapy sessions and implement treatment protocols to enhance physical function.
  • Regularly evaluate patient progress and adjust treatment plans as necessary to ensure effective outcomes.
  • Educate patients and their families about treatment options, home exercises, and wellness strategies.
  • Document patient assessments, treatment plans, and progress in accordance with company policies and regulatory requirements.
  • Collaborate with healthcare teams to ensure comprehensive patient care and communication.
RequirementsQualifications:
  • Must hold a valid and active Physical Therapist license in the state of Michigan.
  • Experience in a home health or rehabilitation setting is highly preferred.
  • Strong clinical skills and a deep understanding of physical therapy practices.
  • Excellent communication abilities and a patient-centered approach.
  • Ability to work independently and manage time efficiently in a home setting.
  • Valid driver’s license and reliable transportation for traveling to patient locations.
