Our mission is to help residents stay healthier, longer—reducing hospitalizations and easing the care coordination burden on facilities and families.Position Summary:Twin Cities Physicians is seeking a Nurse Practitioner (AGNP or CNP) to join our collaborative care team. This role is perfect for a provider who enjoys building lasting relationships, working independently, and serving older adults in their communities. Wound care certification is highly desired but not required—we’ll cover the cost of certification for qualified candidates. This is a fantastic opportunity to grow your clinical skillset while supporting a population in need of advanced care.Key Responsibilities:
Travel to senior living communities to deliver on-site primary care
Manage chronic conditions, provide advanced treatments, and engage in end-of-life planning
Coordinate care with interdisciplinary teams, reconcile medications, and order labs
Communicate effectively with physicians, support staff, facility personnel, and patient families
Qualifications:
Licensed as a Certified Nurse Practitioner (CNP) in Minnesota
AGNP certification preferred
Experience in geriatrics and/or primary care
Valid driver’s license and current auto insurance
Excellent interpersonal and communication skills
Passion for serving aging and vulnerable populations
