What You Will Do
  • Prepares patients for clinical visits, obtaining comprehensive medical histories, medication lists, and vital signs (e.g., blood pressure, pulse, temperature, height, weight).
  • Assists with routine diagnostic tests and procedures as directed by the clinical team.
  • Performs blood draws with precision and care (Phlebotomy), ensuring proper handling of specimens according to protocols.
  • Travels to patient homes or care facilities, adapting to diverse environments (e.g., multi-family residences, facilities with pets, walk-ups).
  • Transports and operates required medical equipment and supplies (up to 30 lbs.) using VNS Health-approved carrying cases.
  • Conducts exam and sterilizes equipment in compliance with infection control standards.
  • Assists clinical staff with virtual patient exam and minor procedures, utilizing virtual equipment.
  • Maintains quality standards to ensure the equipment functions accurately based on manufacturing guidelines.
  • Accurately documents patient information, clinical updates, and encounters in the electronic medical record (EMR) system.
  • Ensures all required documentation is complete, timely, properly identified, and includes necessary signatures and dates.
  • Participates in interdisciplinary team meetings, case conferences, and clinical supervision sessions to ensure continuity of care.
  • Provides services in accordance with all regulatory and compliance standards.
  • Assists Clinician in reinforcing patient and family education about care instructions, medication adherence, and preventative health practices under the guidance of clinical staff.
  • Supports daily operational needs of the VNS Health-affiliated Medical Care at Home practice, ensuring goals and performance targets are met.
  • Schedules patient appointments, manages follow-up reminders, and responds to routine inquiries in a timely, professional manner.
  • Facilitates communication with the patient’s Care Manager and/or Primary Care Provider (PCP) by preparing referral letters, clinician visit notes, and follow-up documentation.
  • Participates in special projects and performs other duties as assigned.
Qualifications:Licenses and Certifications:
  • Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) required
  • NYS Phlebotomy certification required
Education:
  • Completion of a Medical Assistant program required
  • High School Diploma or equivalent preferred
Work Experience:
  • One year of experience in a medical practice, home care, or similar setting required
  • Strong interpersonal, organizational, and multitasking skills. Proficiency in EMR systems and Microsoft Office Suite preferred
  • Skilled multitasker with a patient-focused mindset who thrives in providing patient-centered care preferred
Pay Range: USD $23.17 - USD $28.96 /Hr.
