🕒 Schedule: Exempt, min 40 hours per week | 8:00am - 4:30pm | Monday - Friday🏢 Department: Social Work | LifeCircles PACE | Muskegon Mi🎯 What You’ll Do in This Role:In this role, you’ll be an essential part of a mission-driven team dedicated to enhancing the lives of older adults. You’ll bring compassion, professionalism, and purpose to your daily work, whether you're coordinating care, supporting team collaboration, or directly engaging with participants and their families. Your ability to build trust, think proactively, and uphold high standards will help redefine aging and create meaningful impact in the community we serve.Primary Responsibilities:Interdisciplinary Team Consultation, Staff Training, and Care Partner Coaching
  • Collaborate with interdisciplinary team members to ensure comprehensive, integrated care that aligns medical and behavioral health strategies.
  • Train staff in evidence-based mental health approaches to elevate overall service quality.
  • Coach care partners to strengthen their ability to support participants with behavioral health needs.
Bio-psychosocial and Differential Diagnostic Assessment
  • Conduct thorough bio-psychosocial assessments to understand participants’ full range of needs.
  • Perform differential diagnoses to accurately identify mental health conditions and guide individualized care planning.
Behavioral Health Consultation and Treatment Planning
  • Offer behavioral health expertise to guide the development of personalized, wellness-focused treatment plans.
  • Create care strategies that promote autonomy and support participants in remaining safely within their communities.
Direct Behavioral Health Services
  • Provide individual and group therapy across diverse settings including day centers, participant homes, acute care, and long-term care environments.
  • Deliver therapeutic interventions tailored to meet each participant’s mental health goals.
Program Development and Quality Improvement
  • Lead and support initiatives to enhance behavioral health programming and person-centered care models.
  • Implement data-informed quality improvement strategies that benefit participants, care partners, and interdisciplinary staff.
✅ What You’ll Need:
  • Master’s degree in a behavioral health field such as Counseling Psychology, Clinical Psychology, Applied Psychology, Social Work, Marriage & Family Therapy, or Mental Health.
  • Licensed or license-eligible in the State of Michigan (e.g., LMSW, LLMSW, LPC, LLP, LMFT, etc.).
  • CPR certification or willingness to obtain within 6 months of employment.
  • Demonstrated experience providing behavioral health services, including assessments, therapy, counseling, and behavioral consultation.
  • Experience working with older adults, individuals with serious mental illness, and/or people with disabilities.
  • Solid understanding of evidence-based practices and comfort working in integrated care environments.
  • Ability to accurately identify differential diagnoses and create effective treatment plans.
  • Preferred: Experience with substance use treatment, dialectical behavior therapy (DBT), or caregiver training.
  • Preferred: Previous work in medical or interdisciplinary care team settings is strongly
