This role requires a results-driven, outgoing, and assertive individual with the ability to perform in-field outreach, develop strong relationships, and manage a client recruitment pipeline. The coordinator will serve as a brand ambassador for HeartShare, ensuring our mission and services are represented with professionalism, urgency, and integrity.Primary Responsibilities and Essential Functions of Position:Collaborate with the Program Director to identify and develop new referral partnerships with community-based organizations and health and social service providersAggressively pursue and enroll a minimum of 30 eligible clients per month into the Health Home program through proactive community outreach, direct engagement, and consistent follow-upConduct outreach and engagement through field and home visits, phone calls, and text messages to ensure client contact and completion of the enrollment processMaintain a strong tracking system for all recruitment activities including lead status, follow-ups, outreach methods, and enrollment outcomesOrganize and deliver presentations to external stakeholders about HeartShare's services and enrollment process to foster partnerships and client recruitmentTroubleshoot and resolve partner or client engagement issues quickly and professionally, maintaining a strong customer service ethicMonitor and adjust recruitment strategies in response to performance trends and evolving community needsIdentify barriers to engagement and develop client-centered solutions that address social determinants impacting enrollmentMaintain detailed knowledge of HeartShare's Health Home model and eligibility criteriaProvide accurate and timely documentation and reporting, including referral logs, enrollment data, outreach summaries, and weekly/monthly status updatesAdhere to all regulatory and funder guidelinesCore CompetenciesPersistence and Follow-Through: Consistently follows up with clients and partners to achieve recruitment goalsField Engagement: Demonstrates comfort and success conducting community outreach and client home visitsRelationship Building: Establishes rapport quickly and fosters long-term partnershipsGoal-Oriented: Maintains focus on measurable enrollment targets and recruitment milestonesProblem Solving: Responds quickly to challenges with effective, sustainable solutionsCommunication: Strong in verbal, written, and public speaking skillsQualifications
  • Associate's degree Required
  • Valid Driver License Required
  • Minimum two years of experience in outreach, sales, business development, or social services. Preferred
  • The ideal candidate will have excellent organizational and communication skills, as well as the ability to read, write, and speak English fluently.
