Job Summary:The MDS Nurse manages the process of assessment, planning, facilitation, coordination, evaluation, and advocacy for services and options to meet the individual resident need promote quality, cost-effective outcomes while adhering to Federal and State guidelines, and in accordance with company's policies and procedures.ESSENTIAL JOB FUNCTIONS:1. Manages the RAI Process from resident admission to discharge in order to maintain clinical compliance and receive appropriate funding from Medicare, Medicaid and Managed Care pay sources2. Completes MDS and all pertinent forms required in a timely, accurate, documentation-supported and case mix optimized manner3. Develops, with input from the interdisciplinary team members, individualized care plans reflective of the resident status4. Coordinates resident assessments and services by other clinical departments5. In conjunction with the Business Office Manager, provides timely information for all managed care clinical authorization and justifications6. Manages the daily PPS/OBRA case management meetings and weekly level of care meetings7. Other duties as assignedRequirements
  • Current Texas RN or LPN with at least 2 years experience completing and submitting MDS, Resident care plans in a nursing home environment.
  • Strong working knowledge of PDPM and Skilled Nursing
  • Demonstrated ability to perform concurrent MDS reviews, assuring appropriate RUG category achievement.
