Works in coordination with the Comprehensive Center Stroke Program Manager (or other leader where applicable) and other members of the stroke team to ensure all patients of the institution receive high quality evidence-based care through the provision of expert nursing consultation and practice oversight. In concert with the stroke team, develops and delivers acute stroke continuing education programs. Coordinates and participates in the performance improvement process and stroke research.ResponsibilitiesA day in the life of a Stroke Program Coordinator at Hackensack Meridian includes:
  • Performing gap analyses that explore and clarify areas for program improvement.
  • Leads preparation and attainment of certification, often in collaboration with regulatory, quality and patient safety, and nursing leadership.
  • Collaborates with the quality management department & stroke registry staff to ensure timely and accurate data abstraction/entry occurs.
  • Responsible for ensuring interpretation, execution and compliance with standards for stroke center certification.
  • Identifies and maintains a concurrent list of inhouse stroke patients and monitors clinical progress.
  • Evaluates customer satisfaction and implements processes to establish improvements.
  • Leads preparation and attainment of certification, often in collaboration with regulatory, quality and patient safety, and nursing leadership.
  • Collaborates with the quality management department & stroke registry staff to ensure timely and accurate data abstraction/entry occurs.
  • Monitors compliance to hospital policies, procedures and standards through clinical observations, medical record review, staff evaluation, feedback and other sources.
  • Actively participates on local, regional, and national advisory groups to work to advance the science of care for the stroke patient.
  • Identifies nursing research problems and utilizes nursing research strategies to promote clinical knowledge development and research based clinical practice.
  • Responding to and supporting provision of excellent emergent acute stroke care (Inhouse and ED Code Stroke, as needed).
  • Participates in the review of patient transfers to ensure compliance with clinical practice guidelines and develops corrective actions as needed.
  • Developing evidence-based policies and procedures.
  • Collaborates with Comprehensive Stroke Program Manager (or other leader where applicable) at hub site to ensure programmatic alignment and assists peer sites as needed.
  • Developing and ensuring appropriate use of patient education materials.
  • Engages in both formal and informal bedside education that ensures clinical competency.
  • Responsible for developing, coordinating, and ensuring delivery of regular stroke education to nurses, physicians, clinicians, patients, families, Emergency Medical Services (EMS), communities, referring hospitals.
  • Provides concurrent and retrospective review and analysis of the care of stroke patients at the patient level, program level and institutional level.
  • Utilizes the stroke registry and other data platforms (EA&I, Vizient, Looker, IQVIA) to develop robust dashboard to monitor clinical effectiveness and patient outcomes.
  • Analyzing quality performance data.
  • Collaborates with all members of the stroke team to evaluate and address specific patient care, educational and safety issues.
  • Adheres to HMH Organizational competencies and standards of behavior.
  • Other duties and/or projects as assigned.
QualificationsEducation, Knowledge, Skills and Abilities Required:
  • Bachelor's Degree in Nursing.
  • A minimum of 3 years of clinical hospital experience, preferably in neurology, intensive care, or emergency care.
  • Strong presentation and facilitation skills
  • Excellent written and verbal communication skills
  • Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
Licenses and Certifications Required:
  • NJ State Professional Registered Nurse License.
  • National neuro certification within one year of hire (SCRN, CNRN, NVRN)
Licenses and Certifications Preferred:
  • Certified Neuroscience Registered Nurse.
  • Advanced Cardiac Life Support Certification.
  • AHA Basic Health Care Life Support HCP Certification.
