Physical Therapist Assistant - Home HealthJoin our team as a dedicated Physical Therapist Assistant in Home Health, where you will work under the supervision of physical therapists to help patients regain movement and manage pain in their own homes.Key Responsibilities:
  • Assist physical therapists in providing physical therapy treatments and procedures as outlined in the care plan.
  • Observe and document patients’ status before, during, and after therapy, reporting progress and concerns to the supervising physical therapist.
  • Help patients perform specific exercises and use therapeutic equipment safely in the home environment.
  • Educate patients and family members about exercises and activities to continue between visits.
  • Adapt interventions as needed within the scope of the treatment plan and state laws.
  • Ensure all documentation is accurate and completed in a timely manner according to agency and regulatory standards.
Work Environment:
  • The role is based in patients’ homes, requiring travel within the community and adaptability to various home settings.
  • This position requires strong interpersonal skills and the ability to work independently while supporting patient goals.
