Physical Therapist - Long-Term Acute Care (LTAC)If you are passionate about helping patients recover from severe illnesses or injuries, join us as a Physical Therapist in a Long-Term Acute Care (LTAC) setting. In this role, you will provide specialized therapy services to improve mobility, strength, and overall functionality for patients requiring extended care.Key Responsibilities:
  • Evaluate patients' physical conditions and develop individualized treatment plans tailored to their recovery needs.
  • Create therapy programs focused on improving mobility, strength, coordination, and pain management during long-term care stays.
  • Collaborate with interdisciplinary teams, including physicians and nurses, to ensure comprehensive patient care plans are implemented effectively.
  • Educate patients on exercises and techniques that support continued recovery after discharge from the LTAC facility.
  • Document patient progress accurately and adjust treatment plans as needed based on clinical outcomes.
