- Be fully responsible for on-call coverage during your 7 days on (nights, days, weekends)
- Respond to urgent patient needs including pain, symptom management, and crisis situations
- Provide telephonic support to families and caregivers with empathy and clarity
- Conduct in-home visits as needed across our 21-county service area
- Coordinate with the IDG to ensure appropriate follow-up and care planning
- Maintain accurate, timely documentation in our electronic health record
- Current MN Registered Nurse license
- Previous hospice, oncology, or palliative care experience required
- Clinical confidence to triage and respond to a wide range of scenarios
- Excellent communication and critical thinking skills
- Ability to self-manage a demanding on-call schedule with professionalism
- Reliable vehicle, valid driver’s license, and comfort driving to homes throughout the metro
- Personal cell phone for work communication (device not provided; reimbursement available)
