Job Category: PhysicianDegree Level: Doctorate DegreeJob Description:Nemours is seeking a Physician for our Hospital at Home program! This Physician will lead daily interdisciplinary rounds and remain available throughout the day for clinical escalations.The Nemours ACaH Physician plays a pivotal role in the development, implementation, and continuous improvement of an innovative program designed to support pediatric patients as they transition from hospital to outpatient care. This physician will collaborate closely with an Advanced Practice Provider (APP) and virtual nurses.Key Responsibilities:
  • Collaborative Practice - Partner with Advanced Practice Providers (APPs) and other healthcare professionals to deliver high-quality care to pediatric patients.
  • Initial Assessment - Conduct comprehensive evaluations of patients and families upon referral or admission to the ACaH program.
  • Ongoing Evaluation - Monitor patient progress and revise the interdisciplinary care plan as needed to ensure optimal outcomes.
  • Care Planning Support - Assist nursing staff in developing and prioritizing individualized care plans based on patient and family needs.
  • Discharge Coordination - Collaborate with nurses and the broader care team to create and implement discharge plans that support safe transitions to home.
  • Patient & Family Education - Provide tailored education to patients and families, addressing identified learning needs to support home-based care.
  • Treatment Compliance Monitoring - Assess adherence to treatment plans through telehealth visits, phone follow-ups, and direct communication with families.
  • Evidence-Based Practice - Share relevant research findings and integrate evidence-based practices into patient care within the ACaH specialty.
  • Conduct daily patient rounds
  • Identify and enroll eligible patients into the ACaH program
  • Deliver follow-up care via telehealth

Qualifications:
  • Ideal applicants will demonstrate strong collaboration skills within an interprofessional team and possess the clinical aptitude to deliver care across both inpatient and outpatient environments
  • Candidates with experience in both inpatient and outpatient pediatric healthcare settings are preferred.
  • Comfort managing pediatric patients with medical complexity and technology dependency is essential.
  • MD or DO
  • Appropriate licensure in the states of practice
  • American Heart BLS
