- Promotes quality of care and services for residents in assigned communities, including theirphysical, emotional, and social well-being.
- Perform and/or review initial and ongoing Comprehensive Assessments for residents/tenants.
- Develop, implement, and update Individualized Service Plans (ISPs) based on assessments andchanging resident conditions.
- Attend and contribute to interdisciplinary care conferences.
- Conduct and document assessments per DHS guidelines such as Self-Administration ofMedication, Psychotropic Medication reviews, Pain assessments, Fall Risk or other communityassigned assessments.
- Assures written physician orders are signed, processed and implemented in each assignedcommunity.
- Document observations, concerns, changes in resident conditions, incidents, and/or accidents,ensuring follow-up until resolution.
- Serve as liaison between residents, resident representatives, and physicians/health care providersregarding resident health care issues.
- Coordinate care for residents returning from hospitals or skilled nursing, ensuring appropriatetransitions and follow-ups.
- Monitor residents' health status and coordinate care with physicians, home health agencies,hospice providers, and other external partners.
- Identify and respond promptly to changes in residents' health conditions, ensuring timelycommunication with physicians and resident representatives.
- Participate in resident admission and discharges.
- Maintains up-to-date knowledge of current standards and best practices in assisted living care. 2
- Collaborates with clinical leadership to develop and implement clinical protocols, policies, andprocedures that ensure compliance with applicable laws and Wisconsin state guidelines.
- Designs and oversees an audit process to review nursing assessments and the development ofindividualized service plans (ISPs) across assigned assisted living communities.
- Maintains oversight of the infection control program, including monitoring employee health andrelated protocols in assigned communities.
- Remains accessible and available via cell phone and serves as 'on call' for clinical emergencies,resident changes in condition, and support of staff performing delegated nursing tasks.
- Ensures adherence to all applicable assisted living licensure regulations.
- Monitors the timeliness and quality of nursing assessments for residents and provides clinicalsupport when needed.
- Provide oversight of medication management systems to ensure safe and effective dispensing.
- Reviews medication administration system and medication documentation at least monthly ineach assigned community
- Ensure that MAR (Medication Administration Records) is accurate and complete.
- Ensure the medication cart is orderly and that all medications are properly packaged and stored.
- Assures that medication administration procedures are followed properly and discuss any neededcorrections, errors, and other follow-up with staff.
- Validates the disposal of medications policy is followed at each assigned community.
- Collaborates with senior leadership and community Executive Directors to promote the alignmentof nursing practices with organizational goals and standards.
- Delegate nursing tasks appropriately per the Wisconsin Nurse Practice Act and supervisedelegated tasks to ensure safe and effective care.
- Ensure proper training, competency validation, and ongoing support for direct care staff. • Train and support facility RNs, LPNs, and care staff on clinical protocols and regulatoryrequirements.
- Conduct in-service training on topics such as infection control, medication management,emergency response, and chronic disease management.
- Ensure facility compliance with Wisconsin DHS Chapter 83 (CBRF) and DHS Chapter 89(RCAC) regulations.
- Monitor and assist with incident reporting, follow-up, and mitigation strategies.
- Prepares for and participates in the regulatory survey process.
- Participates in quality improvement initiatives to enhance resident clinical outcomes,quality of life, and operational efficiency.
- Works with community leadership and senior leadership to manage participation in WiCAL'sPEAL program (Performance Excellence in Assisted Living) and Wisconsin Coalition forCollaborative Excellence in Assisted Living (WCCEAL) involvement. 3
- Monitors and encourages the participation of assigned communities in the Wisconsin Board onAging and Long-Term Care (WI BAL) forum meetings.
- Shares relevant information and insights from WI BAL forums with other team members asappropriate.
- Work closely with Executive Directors, care teams, families, and external providers to coordinateresident care.
- Work closely with Regional Operations, Executive Directors, and Support Center leadership tosupport high-quality, efficient service delivery.
- Monitors resident and family concerns to validate prompt and effective resolution.
- Provides input and/or helps perform annual evaluations for nursing and direct care staff.

- o Use and/or disclose only the minimum amount of Protected Health Information necessaryto complete assigned tasks
- o Report all suspected violations of the company's HIPAA policies or procedures to NorthShore Health Compliance Department.
- Active Registered Nurse (RN) licensed in the State of Wisconsin or a valid Multi-Statelicense under the Nurse Licensure Compact (NLC), with Wisconsin as the primary stateof residence or eligibility to practice in Wisconsin under the compact.
- Demonstrated nursing management experience in a resident/patient care setting
- Experience working with older adults in senior living, long-term care, home health orother health care setting preferred or has demonstrated ability or strong desire to engagewith residents with dementia
- Must be able to adapt to a flexible work schedule and occasional overnight travel
- Ability to work in a team environment. Strong communication and interpersonal skills
- Empathy for the challenges experienced by residents, families, and team members and problemsolving skill sets to support them
- Strong computer skills and ability to interact with a variety of electronic devices
- Must have and maintain current CPR certification
- Previous experience in an assisted living setting
- Demonstrated nursing management experience in a resident/patient care setting
- Demonstrated success leading a fun and productive work environment
- 3+ years management experience preferred
