Primary Healthcare Centers is seeking a Licensed Behavioral Health Specialist (LPC, LMFT, or LCSW) to join our integrated care team. This role plays a vital part in improving patient wellness by providing counseling, crisis intervention, and brief behavioral consultations in a primary care setting. The ideal candidate is patient-centered, collaborative, and passionate about making a difference in the lives of others.Key Responsibilities:
  • Deliver individual therapy, behavioral consultations, and psychoeducation using evidence-based practices.
  • Collaborate with providers and clinical staff to create and support patient-centered care plans.
  • Assess and address mental, emotional, and social needs, including crisis intervention when necessary.
  • Provide referrals, coordinate care, and support patients with chronic conditions.
  • Maintain accurate and timely documentation in the EHR.
  • Participate in outreach events and community engagement activities.
  • Support continuous improvement of integrated behavioral health practices.
  • Provide leadership and mentorship to other behavioral health staff if serving in a lead role.
Qualifications:
  • Master’s degree in a human services field (required).
  • Active LPC, LMFT, or LCSW licensure in the state of Georgia (required).
  • Minimum of 5 years' experience in behavioral health; experience in primary care preferred.
  • Proficient in motivational interviewing and evidence-based therapeutic interventions.
  • Strong documentation, communication, and collaboration skills.
  • Ability to assess and respond to crisis situations effectively.
  • Experience with EHR systems and Microsoft Office Suite.
  • Valid driver’s license required.
