Primary Healthcare Centers is seeking a Licensed Behavioral Health Specialist (LPC, LMFT, or LCSW) to join our integrated care team. This role plays a vital part in improving patient wellness by providing counseling, crisis intervention, and brief behavioral consultations in a primary care setting. The ideal candidate is patient-centered, collaborative, and passionate about making a difference in the lives of others.Key Responsibilities:
- Deliver individual therapy, behavioral consultations, and psychoeducation using evidence-based practices.
- Collaborate with providers and clinical staff to create and support patient-centered care plans.
- Assess and address mental, emotional, and social needs, including crisis intervention when necessary.
- Provide referrals, coordinate care, and support patients with chronic conditions.
- Maintain accurate and timely documentation in the EHR.
- Participate in outreach events and community engagement activities.
- Support continuous improvement of integrated behavioral health practices.
- Provide leadership and mentorship to other behavioral health staff if serving in a lead role.
- Master’s degree in a human services field (required).
- Active LPC, LMFT, or LCSW licensure in the state of Georgia (required).
- Minimum of 5 years' experience in behavioral health; experience in primary care preferred.
- Proficient in motivational interviewing and evidence-based therapeutic interventions.
- Strong documentation, communication, and collaboration skills.
- Ability to assess and respond to crisis situations effectively.
- Experience with EHR systems and Microsoft Office Suite.
- Valid driver’s license required.
- Patient populations served may include adults, geriatrics, children, and adolescents.
- Some remote work may be permitted.
