The ideal candidate for this position will share our values and commitment to providing exceptional care while promoting health, safety, and independence.As a PRN Home Health Physical Therapist Assistant at AdvisaCare, you will work closely with patients in their homes, providing assistance with their treatment plans under the direction of a licensed Physical Therapist. You will be an essential part of our team, helping to support the care of individuals and families within the community in which you live!Responsibilities
  • Implement physical therapy treatment plans as directed by a licensed Physical Therapist
  • Prepare and maintain accurate and timely clinical documentation
  • Assist patients with exercises and activities to promote strength, safety, and mobility
  • Provide education to patients and their families about their conditions and treatment plans
  • Ensure a safe and effective home environment for clients
  • Collaborate with interdisciplinary team members, including nurses, physical therapists and occupational therapists, to coordinate patient care
Requirements:
  • Current state licensure in Michigan as a Physical Therapist Assistant
  • Minimum of one year of experience in home health care is preferred, but not required
  • Strong communication and interpersonal skills
  • Ability to work independently and manage multiple patients effectively
  • Valid driver's license and reliable transportation
