Position Summary:Assist the HC Administrator, Director of Nursing (DON) and Assistant Director of Nursing (ADON) with planning and implementing objectives, policies and procedures for resident care services, in accordance with federal and state regulations, for the Abbey Memory Support Villa Assisted Living residents. Provide direct supervision and general oversight to team members who provide resident care services in the Abbey and Villa.Essential Functions:
  • Lead and manage Villa and Abbey Care Partners to assure the desired level of resident service, care, and satisfaction. Monitor and evaluate team member performance; provide coaching, training, and corrective actions as required. Assure equitable administration of policies, and compliance with regulatory and legal requirements. Develop and provide appropriate recommendations for the full range of team member and organizational matters, including staffing, compensation, promotion, and termination actions.
  • Conduct regular team member meetings according to quality care standards and practice guidelines. Provide for professional growth and development of team members through identification of needs, development of and participation in individual training plans, and encouragement of formal continuing education efforts
  • Communicate federal and state regulatory requirements, and any changes as appropriate, to team members to assure that high quality care is maintained for Abbey/Villa residents and annual surveys reflect compliance with regulations.
  • Conduct regular rounds to monitor residents' activity and quality of care that the team member are providing; and to monitor for any indication of abuse, mistreatment or neglect and take corrective actions as appropriate.
  • Assist the InterDisciplinary Team in developing individual resident care plans and the team members in implementing those care plans that include rehabilitative and restorative activities to assure that the care that residents receive is appropriate, monitored and provided in a timely manner.
  • Support a dynamic Quality Assurance Performance Improvement (QAPI) program for the Abbey/Villa that effectively identifies problem areas (i.e. infection control, weight loss etc.) and provides tracking and trending of issues through effective resolution.
  • In coordination with ADON/DON/Admin assure that reportable incidents are properly investigated and documented and reported timely to the Oklahoma State Department of Health (OSDH) and the Department of Human Services (DHS) to assure Montereau's compliance with regulations.
  • Participate in routine meetings to address immediate issues and coordinate changes within the organization that may affect Abbey/Villa residents or its operations.
  • Developing and maintaining resident and family relationships for continuous satisfaction.
  • Assisting with census development relating to referral relationships, giving tours and prospect communication to potential residents and family members
Other Functions:
  • Participate in the Manager on Duty (MOD) rotation.
  • Work floor shifts when needed due to a temporary staff shortage in order to assure proper resident care is provided.
  • Perform other related functions as required or as assigned.
Position Requirements:Experience:
  • A minimum of 3 years of experience within a long-term care (LTC), skilled nursing, assisted living or hospital environment is required.
  • Previous experience working with federal, state and local resident care regulations and practical experience analyzing and interpreting statistical reports while working in an interdisciplinary team environment is required,
  • Previous experience within a Life Plan Community is preferred.
  • Previous leadership experience in a health care setting is preferred.
Education/Certifications/Licenses:
  • Bachelor degree in a Health Care field is preferred
Job Specific Knowledge, Skills, and Abilities:
  • Understanding of state and federal regulations relating to Assisted Living
  • Strong resident care documentation skills
  • Working knowledge of Microsoft Office programs and ability to learn new software systems (i.e. AOD)
  • Working knowledge of Minimum Data Sets (MDS) is preferred
  • Strong communication skills to effectively and empathetically address critical health and end of life issues with a diverse resident population and family members
  • Solid problem-solving skills and decision making skills
  • Strong interpersonal skills and understanding of quality customer management and service
  • Ability to effectively interact with a diverse leadership team, team members, residents, and third-party associates, including vendors, providers, and visitors
  • Ability to effectively manage sensitive and confidential communications and information, while maintaining a high level of poise, tact and diplomacy.
Physical Demands:In order to accomplish the essential functions of this position, the following are required:
  • An extensive amount of walking, standing, and sitting, with a moderate amount of bending at the waist, kneeling, twisting/turning, reaching above the head and shoulders, and eye-hand coordination
  • Ability to routinely lift 25 pounds and push wheelchairs with loads of up to 300 pounds
  • Frequent use of ordinary communication, including normal levels of hearing and vision, including both near and far seeing, color discrimination, field of vision, and depth perception
  • Ability to use a computer or other office equipment for an extensive period of time
  • Ability to occasionally travel by car and/or commercial airline
Working Conditions:This position is performed primarily indoors in both a private office environment and in a nursing resident environment with occasional exposure to high noise levels (alarms).Mental and Emotional Requirements:This position requires:
  • Understanding and carrying out oral and written instructions and reviewing/preparing detailed reports, schedules and medical records, using measuring devices and simple calculations
  • Planning own work activities and the activities of others, and directing and training others in their work assignments
  • Working independently or as a member of a team, and the ability to work with limited supervision
  • Routinely working on multiple tasks at the same time, under moderate amounts of pressure for results, with frequently changing priorities and requirements for extreme accuracy
Safety/Security Requirements:
  • Must successfully pass a pre-employment drug test
  • Must successfully pass a criminal background screening
  • Must successfully pass a tuberculosis (TB) screening test
