SummaryAmbulatory Pharmacist - Anticoagulation ClinicThe successful candidate will practice under a Therapy Management Contract to manage anticoagulation therapy through in person and virtual patient interactions. Responsibilities include targeted and comprehensive medication review, patient counseling and education, disease state and medication therapy management, drug information, and addressing barriers to medication access and adherence. Additional responsibilities include tracking and reporting productivity and quality metrics, involvement in the system-wide AC Clinic Collaborative and design and conduct of quality improvement activities. The schedule for this position includes a rotating weekend staffing commitment in the inpatient setting. Opportunities exist to serve as a mentor and educator for pharmacy residents, pharmacy students, pharmacists and healthcare professionals across other disciplines. Career ladder is in place to encourage continued professional growth and advancement.REQUIREMENTS: Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline. Completion of a PGY-1 Pharmacy Practice Residency or a minimum of three (3) years direct patient care experience in hospital setting where knowledge is applied toward pharmacotherapy, resulting in a measurable patient outcome. 3-5 years of experience.
