Employment Type: Full-TimeA mission-driven Managed Long Term Care (MLTC) program serving the diverse communities of Brooklyn is seeking a Registered Nurse to join its Intake team. This is a hybrid role requiring field-based assessments and in-office collaboration.The MLTC Intake RN plays a vital role in evaluating potential new members for program eligibility, educating them and their families on available services, and supporting a smooth transition into long-term care. Bilingual language skills in Chinese or Spanish are highly preferred due to the populations served.Key Responsibilities:Conduct in-person assessments using the UAS-NY tool to determine program eligibility and safety for potential members.Educate prospective members and families about the MLTC program, services, and enrollment process.Collaborate with the Intake/Outreach and Clinical teams to coordinate timely, appropriate onboarding of new members.Communicate professionally with referral sources, physicians, caregivers, and community partners.Participate in outreach activities to support community engagement and referrals.Qualifications:Active New York State RN license required.Bachelor’s degree in Nursing required.Minimum 1 year of acute care experience.Proficiency in UAS-NY assessments required.Experience in home care and care management preferred.Strong communication and interpersonal skills; fluency in Spanish or Chinese is highly desirable.Proficiency in Microsoft Office applications.Join a collaborative team making a direct impact in the lives of vulnerable New Yorkers.
