Job OpportunityJob ID: 49916 Positions Location: Lansing, MI Job Description General Purpose of Job:Description:Positions Location: Lansing, MIJob DescriptionGeneral Purpose of Job:Responsible for assessing, planning, implementing, and evaluating patients in collaboration with the hospice and palliative care physicians.Essential Duties:This job description is intended to cover the minimum essential duties assigned on a regular basis. Caregivers may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.
  • Assesses, plans, treats, and evaluates care for patients with palliative care-based needs.
  • Collaborates with health care team and coordinates care for patients.
  • Organizes and conducts patient/family case conferences.
  • Conducts informed consents with patients considering Hospice Care.
  • Works closely with physicians, associates, patients, and families to provide quality care to patients with serious or life-limiting illness.
  • Attends Interdisciplinary Team meetings.
Job Requirements General Requirements • Current State of Michigan registered nurse license· • Current State of Michigan certified nurse practitioner license
Work Experience • Minimum of two years experience with either palliative care, hospice, critical care, case management, or oncology.
Education • Masters of Science in Nursing
Specialized Knowledge and Skills • Must have strong interpersonal, communication, and problem solving skills • Must be well organized and able to manage time effectivelyThe University of Michigan Health - Sparrow Lansing is seeking an enthusiastic and dedicated part-time PA/NP to join their Palliative and Hospice team .   • 28 hours per week
  • Clinic and inpatient
  • Call 5- 6 weekends per year
  • Preferred 1 year experience in Hospice and/or Palliative Care
  • EPIC EMR
