- Provide supervision, mentorship, and support to nursing staff to promote professional development and optimal patient outcomes.
- Collaborate with interdisciplinary teams to create and implement personalized patient care plans.
- Ensure precise and timely documentation of patient care and progress.
- Conduct regular audits of patient records and care plans for quality assurance.
- Serve as a communication link between patients, families, and healthcare providers to ensure seamless care coordination.
- Engage in continuous quality improvement initiatives and professional development activities.
- Previous experience in home care setting with two years management or supervisory experience, preferred.
- Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
- Registered nurse with current licensure to practice nursing in the practicing state.
