- Identify, assess, plan, implement, coordinate, monitor, and evaluate healthcare services to meet an individual patient's specific health care needs in a cost-effective manner. (may include but not limited to home health assessment, telehealth initiation, disease management education, coordinate of post-acute services)
- In collaboration with the Community Health Worker, coordinate patient care with patient, family, staff, physicians, and ancillary/community services to promote quality and cost-efficient care for assigned caseload.
- Ensure weekly contact with patient (in person or via telephone) to progress towards care plan goals
- In collaboration with the Community Health Worker, act as an advocate for the connection with community resources.
- Conduct patient home assessment visit when indicated
- Graduate of an accredited school of nursing with a minimum of an associate of science degree required, bachelor’s degree preferred
- 1 - 3 years’ experience in acute care setting at least 1 year of prior experience as case manager, discharge planner or care coordinator required
- Maryland RN licensure required
- Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance required
- Active American Heart Association Basic Life Support (BLS) certification required
