Job Description:Nye Health Services, a family-owned company that has been recognized nationally for our care, service, and culture is seeking an MDS Clinical Coordinator (RN or LPN) for our Nye Pointe campus located in Fremont, NE . This role reports to the Director of Nursing and is responsible for the MDS and care planning activities/systems for the campus, infection prevention, and the execution of clinical education in compliance with all applicable state and federal regulations.   • Develop, supervise, plan, coordinate, organize, direct, evaluate, and monitor the MDS, CAAs, and care planning process on the campus.
  • Complete administrative duties such as completing appropriate forms, inspections, reports, OSCAR reports, assessments, MDSs, CAAs, care plans, progress notes, and similar items as assigned.
  • Assist the Director of Nursing in supervising, coordinating, directing, and monitoring the staffing of the Nursing department.
  • Keep apprised of and implement regulations, trends, and developments in nursing-related areas.
  • Assist in survey compliance, quality assurance and team member development duties, as assigned.
  • Supervise the training of and delivery of cares.
Qualified candidates will have:
  • Graduate from an accredited school of Nursing approved by the State of Nebraska.
  • Will be willing and able to act in an on-call capacity in a shared rotation.
  • Possess a valid RN or LPN license from the appropriate state. This shall include maintaining required CEU's.
  • Experience in Long Term or Acute Care and MDS/RAPS/Care Planning preferred, but not required.
  • Must be willing and able to become CPR and first aid certified.
