We are seeking a dedicated Home Health Physical Therapist to provide personalized, evidence-based therapy to patients in their homes. This role focuses on improving mobility, independence, and overall quality of life, while collaborating with an interdisciplinary team to ensure comprehensive, patient-centered care. The ideal candidate will have strong clinical skills, a patient-first mindset, and the ability to manage a diverse caseload efficiently.Key ResponsibilitiesPatient Care Excellence
- Evaluate patients and develop individualized treatment plans focused on mobility, independence, and functional improvement.
- Deliver high-quality, evidence-based therapy interventions in the home environment.
- Educate patients and caregivers on exercises and strategies to promote recovery and prevent injury.
- Adjust therapy plans as needed based on patient progress and evolving needs.
- Maintain thorough and timely documentation in the electronic health record (EHR) to ensure compliance and care continuity.
- Monitor outcomes and track progress toward established goals.
- Work closely with nurses, social workers, and other therapists to provide coordinated, holistic care.
- Participate in interdisciplinary team meetings to review patient progress and align care plans.
- Mentor therapy assistants or junior clinicians as needed.
- Bachelors or Doctorate degree in Physical Therapy (DPT) from an accredited program.
- Active state licensure as a Physical Therapist.
- Minimum 2 years of physical therapy experience, preferably in home health or a similar setting.
- Strong knowledge of evidence-based therapy practices and home health regulations.
- Excellent communication, problem-solving, and time management skills.
- Proficiency in EHR systems and telehealth tools.
- Familiarity with Medicare, Medicaid, and state-specific home health guidelines.
- Primarily provides care in patients homes, with occasional office visits for meetings or training.
- Regular travel within the assigned service area is required.
- Dynamic, patient-centered environment requiring adaptability, organizational skills, and the ability to balance multiple priorities.
