As a Clinical Coordinator, you’ll be at the heart of that impact—helping to build strong, collaborative teams that deliver exceptional care.Schedule: Monday through Friday 8:00a to 5:00p (in office, not remote)Why Join Elara Caring?Collaborative, team-oriented environmentCompetitive compensation and benefits (pay is based off experience with a range between $21-23/hr)Tuition reimbursement for full-time staff & free continuing education for all employeesOpportunities for advancement and professional growthComprehensive insurance (medical, dental, vision, pet)401(K) with employer matchGenerous paid time off, holidays, and family/pet bereavement leaveWhat You’ll Do as a Clinical Coordinator:Lead and Support Teams – Build, guide, and support care teams to ensure patients receive the highest quality of life and care.Scheduling & Organization – Manage scheduling systems and communication processes that ensure seamless care delivery.Collaborate Across Departments – Partner with clinical supervisors, physicians, caregivers, and staff to maintain patient-centered care.Ensure Compliance & Quality – Monitor medical records, evaluate operations, and identify opportunities to improve workflows and productivity.Promote Teamwork & Communication – Provide clear, supportive communication to staff, patients, and families while fostering a collaborative work culture.Support Daily Operations – Assist with non-clinical inquiries, maintain departmental organization, and oversee documentation accuracy.Champion Our Mission – Model Elara Caring’s values, uphold HIPAA confidentiality, and contribute to a positive, patient-first environment.What We’re Looking For:Healthcare experience required – 1+ year in hospice, home health, or a related healthcare settingStrong scheduling, organizational, and time-management skillsProven ability to communicate effectively and work collaboratively with clinical teamsKnowledge of medical terminology, Medicare/Medicaid guidelines preferredProficiency with office technology and electronic documentation systemsHigh School Diploma; college degree preferred👉 If you’re ready to combine your healthcare experience, strong scheduling skills, and passion for teamwork into a role that makes a real difference, we’d love to hear from you.
