
Housing Navigator

- Identify and sustain relationships with temporary and affordable housing services as well as hotels/motel, moving companies and housing set-up resources
- Encourage and support the coordination of program participants to find housing, using a Housing First model
- Process and track housing payment transactions per program participant
- Advocate on behalf of clients with landlords and other housing professionals, work with property management personnel to ensure well supported care needed to maintain housing eligibility
- Document housing services using the program databases
- Communicate additional needs identified for each participant to assigned case manager
- Protect client confidentiality following program protocol by utilizing encrypted email and/or fax, and obtain consent to release information for communication with other agency partners
- Administer screenings, or assessments as defined by the program
- Participate in weekly reflective supervision meetings with Program Supervisor/ Manager
- Contribute to program protocol review and modifications
- Communicate participant incidents to Program Supervisor/ Manager before action is taken
- Respond to chart audit recommendations and complete by deadline
- May organize and present educational sessions with participants as needed
- May attend community network and collaborative meetings as assigned
- Participate in outreach activities or provide a program presentation to agency partners
- High school graduate or equivalent
- Minimum of 4 years’ experience in case management, housing navigation, social work or equivalent
- Valid California driver’s license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes; minimum two years’ driving experience; no more than 2 violations or a single ticket/accident value at more than 1 point on driving record
- Availability and willingness to work evening and weekend hours, as required
- Bachelor's degree in related field with two years’ case management or social work experience
- Bilingual (English/Spanish)
- Experience working with housing navigation
- Expertise of relevant resources within the community
- Ability to demonstrate knowledge of trauma-informed care and practices
- Excellent verbal and written skills necessary for communication with patients/clients, providers and other staff
- Ability to interface with all levels of personnel in a professional manner, including people of all social, cultural and ethnic backgrounds and within the constraints of government funded programs and those community members facing justice involvement
- Ability to prioritize and meet deadlines
- Ability and willingness to meet the organization’s attendance and dress code policies
- Ability to handle confidential materials and information in a professional manner
- Excellent customer service skills and commitment to providing the highest level of customer satisfaction
- Experience/familiarity with computers, business e-mail, communication systems and internet search capabilities and proficient in Microsoft Office products, specifically Word and Excel
