As our Intake Coordinator, you’ll be the initial point of contact for new home health referrals, ensuring seamless onboarding, regulatory compliance, and a compassionate welcome for every client. This is your chance to make a direct impact on patient care—right from the very first step.Job Description: Championing Patient-Centered OnboardingAs an Intake Coordinator, you’ll manage the intake and recertification of home health referrals, collaborating with physicians, patients, and our care team to ensure the highest standards of accuracy, communication, and compliance. Your role is critical in setting the tone for the quality of care our patients and families experience.Key Responsibilities:Referral Management: Obtain and accurately document all referral information fornew and existing home health patients.Order Coordination: Secure and verify physician orders for home care initiation and recertification; communicate orders promptly and clearly to the care team.Care Plan Collaboration: Work closely with physicians and other clinicians to implement and update individualized patient care plans.Documentation & Communication: Maintain thorough, timely, and accurate patient records; communicate patient data, observations, and changes to the care team both verbally and in writing.Patient Advocacy: Support and educate patients and families on care plans, pain management, and agency services.Regulatory Compliance: Ensure all intake and recertification processes meet agency, state, and federal standards.Key Responsibilities Continued:Quality & Safety: Promote a safe care environment by adhering to best practices inmedication administration, infection control, and risk reduction.Team Support: Mentor and support new staff in the intake process; participate in performance improvement and compliance initiatives.Candidate Profile: Compassionate, Organized, and Detail-OrientedLicensure: Current RN or LPN license in Virginia (required)Experience: Minimum 1 year in a clinical, home health, or intake coordination role preferredCommunication: Exceptional verbal and written communication skills with a strong focus on patient and family educationIndependence: Ability to work autonomously, prioritize tasks, and manage time effectivelyTechnical Skills: Proficient in EMR/documentation systems and comfortable with technologyProfessionalism: Maintains confidentiality, complies with all agency policies, and represents Charity Care Group with integrity and empathyPhysical & Environmental DemandsAbility to sit, stand, walk, and use hands to handle or feel for extended periodsOccasional lifting (up to 25 pounds)Frequent talking, hearing, and computer useWork is performed in a clinical office and/or patient home environmentWhy Join Charity Care Group?Mission-Driven Work: Make a meaningful difference in the lives of patients andfamilies from the very first interactionSupportive Culture: Join a team that values compassion, excellence, and professional growthWork-Life Balance: Flexible scheduling and a calm, collaborative work environmentImpact: Be the crucial bridge between referral and care, ensuring every patient receives the best possible startReady to help patients start their care journey with confidence and compassion?
