SUMMARYProvides support and supervision to adults with developmental disabilities in their home and while participating as active members in their community. Provides assistance/support with personal care, household related duties, skill building, and developing relationships with others following a plan of care. Documents and communicates resident progress and needs. Monitors/communicates the ongoing health and safety needs of residents.ESSENTIAL FUNCTIONS: * Supports residents with basic needs including: Self-care, bathing, and toileting; household chores and meal planning/preparation, modeling appropriate social behavior; communication; basic financial management, and integrating with others outside of the home.
  • Supports residents with and in social and leisure activities inside and outside the home.
  • Assists residents with physician appointments.
  • Assists with transporting residents.
  • Administers and documents the administration of resident medications.
  • Physically assists residents who may be of limited mobility.
  • Documents daily progress notes.
  • Communicates needs, health concerns, and progress to management and other team members.
  • Acts as a liaison with family members and other concerned parties on behalf of the residents.
  • Greets visitors and supervises visitation in the home.
  • Secures the home and communicates maintenance issues or needs to management.
  • Follows each resident's plan of care in collaboration with resident, his/her family, and other service providers, as appropriate.
  • Provides timely and accurate documentation (daily notes, checklists, reports, etc.).
  • Provides awake overnight supervision to residents, responding to resident needs as necessary during shift.
KNOWLEDGE, SKILLS, AND ABILITIESSome experience working with adults with developmental disabilities and/or mental illness preferred (i.e. knowledge of person-centered practices and techniques). Knowledge of common housekeeping practices (cleaning, cooking, laundry, household related tasks). Ability to support individuals with hygiene related tasks such as bathing and toileting. Ability to communicate effectively, orally and in writing. Ability to establish and maintain effective working relationships with residents, their families, other staff, and significant others. Ability to document accurate participant records. Good physical condition, including the ability to physically assist participants who may be of limited mobility. Ability to follow directions and work independently.FORMAL TRAINING AND WORK EXPERIENCE EXEMPLIFYING KSAsHigh School Diploma or GED. Preferably some experience in the treatment of persons with a developmental disability, or combination of education and experience to produce necessary knowledge, skills, and abilities.LICENSURE: Valid Virginia driver's license.
