
Inpatient Rehabilitation Admission Liaison

- Screening and Assessments: Performs on-site clinical evaluations gathering patients’ medical history, current condition, and therapy progress to determine rehab eligibility
- Collaboration: Works closely with physicians, therapists, nursing and case managers to ensure appropriate discharge planning of patients
- Education and Advocacy: Serves as a resource for internal and external referral sources, patients and/or their support system by educating them on rehab benefits and criteria.
- Coordination: Facilitates smooth transitions between different care settings (hospital, subacute, inpatient rehab, etc.)
- Community Outreach: Builds relationships with hospitals, subacute settings, and other healthcare settings and providers to foster referrals
- Clinical Expertise: Maintains clinical expertise in rehab admission criteria, per CMS, that supports compliance with the Inpatient Rehabilitation Facility Patient Assessment Instrument (IRF-PAI) and supports completion of other CMS required documents
- Bachelor of Science Degree in Nursing
- Current Wisconsin RN License
- BLS Certification
- Minimum of two years’ experience in Acute Care Hospital setting or Inpatient Rehabilitation Facility
- Valid Driver’s license
- Preferred: Experience with Neurological population, hospital admissions, marketing or sales. EMR Navigation (Epic)
- Ability to move freely (standing, stooping, walking, bending, pushing and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance
- Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties
- Frequent exposure to sharp objects and instruments
- Occasional exposure to moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electrical shock.
- Occasional high noise level in work environment.
- Possible exposure to communicable diseases, hazardous materials, and pharmacological agents.
- Occasional contact with aggressive and or combative patients.
- Use of computers throughout work day
- Frequent use of keyboard with repetitive motion of hands, wrist and fingers
- Operation of a motorized vehicle with a valid driver’s license for travel between ThedaCare hospitals.
