SUMMARYThe Occupational Therapy Assistant implements treatment programs individualized to the patients needs and discharge plan, issues and provides instruction in the use of adaptive equipment, and documents patient response to treatment according to department policy.Rochester Regional Health's Park Ridge Living Center has been serving elders in our community since 2010. As a testament to our commitment to excellence, Park Ridge Living Center, which houses Wegman Family Cottages, has achieved and maintained a five-star rating from the Centers for Medicare and Medicaid Services (CMS) since 2013.STATUS: Per DiemLOCATION: Clifton Springs Nursing Home - 2 Coulter Road Clifton Springs, NY 14432DEPARTMENT: Long Term CareSCHEDULE: Monday - Friday | DaysATTRIBUTES:Associates Degree or Certificate in Occupational Therapy or Physical Therapy from an accredited school.New York State Licensed Occupational Therapy AssistantExcellent interpersonal skills.RESPONSIBILITIES:Documentation. Completes and documents assessments, establishing objective, achievable and measurable long and short term goals. Documents patient's response to treatment.Treatment. Implements treatment programs individualized to the patients needs and discharge plan; issues and provides instruction in the use of adaptive equipment. Communicates directly to therapist regarding patient's plan of care, concerns or any observed changes in medical status.Discharge Planning. Participates in discharge planning, performs timely and adequate family and staff teaching to facilitate discharge plan. Offers appropriate suggestions for equipment needs post-discharge. Communicates home programs in neat, concise and organized mannerPHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.PAY RANGE: $30.00 - $30.00The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future.
