You will work closely with a multidisciplinary team of clinicians, including cardiologists, cardiac surgeons, anesthesiologists, nurses, technicians and device company representatives, to ensure seamless and efficient care delivery. Your primary responsibilities will include:1. Patient Management:- Facilitate patient referrals and coordinate appointments for diagnostic testing, consultations, and procedures.- Serve as the main point of contact for patients and their families, providing education and support throughout the treatment process.- Collaborate with clinicians to develop individualized care plans based on patient needs and treatment goals.2. Procedure Coordination:- Coordinate scheduling of structural heart procedures, ensuring all necessary pre-procedural evaluations and preparations are completed.- Arrange the availability of resources, such as equipment, medications, and surgical suites, to support safe and effective procedures.- Communicate with patients, physicians, and other healthcare professionals to ensure clear understanding of procedural details.3. Data Management:- Collect and maintain accurate patient data related to structural heart procedures for documentation, research purposes, and quality improvement initiatives.- Ensure compliance with regulatory requirements regarding data privacy and confidentiality.- Generate reports summarizing procedural outcomes and participate in data analysis for performance improvement projects.4. Team Collaboration:- Collaborate effectively with members of the multidisciplinary team to optimize patient care outcomes through timely communication and coordination.- Prepare Powerpoint presentations for and participate in team meetings by discussing patient cases, sharing knowledge, and contributing to decision-making processes.- Act as a resource person for colleagues by sharing expertise on structural heart procedures and best practices.Qualifications:- PA or NP degree.- Minimum of 2 years experience in the field of Cardiology or Cardiac Surgery is preferred.- Strong organizational and coordination skills to manage multiple patients and procedures concurrently.- Excellent interpersonal and communication skills to interact effectively with patients, families, and healthcare professionals.- Proficiency in medical record documentation and familiarity with electronic health record systems.- Ability to work collaboratively in a team-based environment and maintain a proactive attitude towards professional development and continuous learning.Tenet complies with federal, state, and/or local laws regarding mandatory vaccination of its workforce.
