- Performs and supervises medication distribution, medication inventory control, approved medication cost initiatives and medication outdate management.
- Performs and supervises medication order entry and verification. Actively and accurately implements approved institutional initiatives, policies and pharmacy protocols.
- Provides clinical consult services, patient education and serves as a resource and drug information specialist for healthcare professionals.
- Consistently contributes to the pharmacy team effort by assisting pharmacists and technician and precepting students, residents and new staff.
- Ensures compliance with Federal, State and local laws and regulations following institutional approved policies, procedures and protocols.
- Participates in meeting quality goals and in activities related to hospital and departmental performance improvement. Takes initiative to identify and improve patient care processes.
- Performs proper and timely documentation of patient care and regulatory requirements.
- CRITICAL CARE: Primary responsibility to provide pharmacy services to critical care areas and respond to medical emergencies including codes, stroke and STEMI alerts, and traumas.
- PHARMACOTHERAPY CLINIC: Primary responsibility to provide direct outpatient care through approved collaborative practice agreements and other approved programs.
- OUTPATIENT INFUSION CLINIC: Provide pharmacy services to the outpatient infusion clinic. Provide expertise in the area of chemotherapy and biologic medications. Involved in inventory management and compounding medications. Supervise and perform duties as relating to current USP guidelines.
- ANTIMICROBIAL STEWARDSHIP: Primary responsibility to manage the antimicrobial stewardship to meet regulatory requirements and oversee development of new programs related to infectious disease. Review and monitor antimicrobial therapy for effective use, optimizing outcomes by utilizing appropriate therapy interventions.
- INTERNAL MEDICINE: Primary responsibility to provide pharmacy services to medical, oncology and surgical units.
- OVERNIGHT: Primary responsibility to provide pharmacy services with minimal supervision and respond to medical emergencies including codes and level one traumas.
- Demonstrates effective leadership and sound professional judgment consistent with clinical/academic background
- Ability to assess patient data and provide pharmaceutical care appropriate to the patient's age and condition.
- Excellent oral, written and interpersonal communication skills
- Ability to interact professionally with physicians and other members of the healthcare team
- Commitment to continued competence in clinical problem-solving, judgment, and decision-making; communication; education; medical information evaluation and management; management of patient populations; and a broad range of therapeutic knowledge.
- Competently participate in medical emergencies.
- Flexibility to learn and work in multiple areas to provide pharmacy services.
- PharmD or Bachelor of Science in Pharmacy from an ACPE accredited School of Pharmacy
- Current registration and licensure with the Wyoming Board of Pharmacy or the ability to become licensed upon hire
- Cheyenne Regional AHA RQI within three months of hire.
- ACLS certification within 6 months of hire.
- CRITICAL CARE: PALS certification within 6 months of hire.
- PHARMACOTHERAPY CLINIC: Anticoagulation Therapy Management training within six months of hire.
- ANTIMICROBIAL STEWARDSHIP: The lead antimicrobial stewardship pharmacist must have a nationally recognized certification in infectious diseases per CMS and JCAHO.
- OVERNIGHT: PALS certification within six months of hire.
- Health-system experience
- Completion of an accredited PGY1 residency or equivalent
- Board certification through the Board of Pharmacy Specialties
