- Accepts inquiries and conducts pre-admission reviews of potential patients in a timely manner.
- Documents all pertinent information received during the inquiry process and communicates with other appropriate staff for admission decisions.
- Obtains prior authorization and continued authorizations for placement and treatment of managed care patients.
- Determines the current and future care options for assigned patients and the estimated cost of each option.
- Monitors ongoing treatment plans of assigned patients and communicates with payors to insure appropriate reimbursement.
- Initiates and follows through with discharge planning to insure discharge to appropriate level of care in a timely manner using information obtained from interdisciplinary team and attending physician.
- Communicates with business office staff regarding rates, exclusions, changes in level of care for managed care patients.
- Monitors patient needs and managed care authorized services to insure efficient use of available resources
- Monitors daily all HMO/managed care residents for care needs and physician orders relating to same. Requests and receives level changes throughout resident's stay as appropriate.
- One year experience as Case Manager or Admissions Coordinator
- RN, LVN, BSW or other clinical/medical educational background
- Case Manager certifications preferred
- Knowledge of health insurance and managed care issues
- Marketing experience preferred
- Knowledge of rehabilitation and sub-acute care coverage
