- Bachelor's degree in pharmacy.
- Active Pharmacist license in the State of OH
- Prescription formula and mathematics for calculating dosage and quantities
- Working knowledge of all Ohio Board of Pharmacy regulations
- 1-2 years' experience in Pharmacy
- Creates and maintains an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm, professional environment. Establishes effective collaborative working relationships and open and active communication with pharmacy leadership, physicians, nurses, pharmacists, and technicians. Clearly articulates ideas and thoughts through written and verbal communications.
- Contributes to direct patient care through careful review of patient information, assessment and provision of appropriate pharmaceutical recommendations. Promotes the correct and appropriate use of medications by maximizing the clinical effect, using the most effective treatment, minimizing the risk of treatment-induced adverse events, and minimizing the expenditures for pharmacological treatments. Communicates findings and recommendations to other health care professionals who are responsible for the patient's care. Ensures that issues are resolved before dispensing. Provides education/counseling to patients about drug therapies, and provides concise, timely responses.
- Ensures the 7 patient “rights” are respected: right patient, right dose, right route, right time, right drug with the right information and documentation. Reports medication errors and assists in assessing the risks and stratigies to manage the risk. Ensures compliance with regulatory requirements and organizational policies and procedures.
- Provides a consistent process of patient care to ensure the safety of the patient's medication use. Assists in protocol development, implementation, and optimization.
- Takes the lead in identifying opportunities and solutions in pharmacy process improvements. Applies known solutions to a variety of predictable problems. Assumes personal responsibility for professional development, particularly in the area of ongoing clinical education, and develops clinical pharmacy programs according to policies and regulations. Participates in relevant committees.
- Assists in orientation and training of new hires, and precepts pharmacy students and residents.
- Strong interpersonal, analytical, organization, and problem-solving skills.
- Ability to work collaboratively with diverse multidisciplinary teams.
