- Maintains and updates digital and physical records of all families as required by governing agencies.
- Collaborate with clients to develop service plans based on an assessment of each family’s needs.
- Assist families with referrals to outside agencies for employment, education, parenting, legal, medical, and mental health services.
- Maintain and update case files bi-weekly to document progress.
- Assist families with moving into permanent housing.
- Ensure that clients maintain an open public assistance case.
- Adhere to all city and state guidelines for maintaining documents.
- Conducts unit inspections.
- Bachelor’s or Associate's Degree in Human Services or related field.
- Familiarity with case management is a plus.
- Knowledge of Outlook, Word, and Excel.
- Bilingual in Spanish or other language is a plus.
