This Clinical Pharmacist I (Medication Reconciliation/Transitions of Care) is responsible for maintaining all aspects of medication reconciliation, as well as the transitions of care program which includes review of prior to clinic and prior to admission medication histories, review and counseling of discharge medications, and post discharge follow up. The goal of medication reconciliation and the transitions of care program is to improve communication around the patient's medications, improve patient satisfaction, and reduce medication related adverse events and harm. This individual will supervise technicians and assist with training/education for pharmacy staff. Oversight responsibilities also include but are not limited to overseeing pharmacy technicians' compliance to established processes and productivity goals, as well as documenting key performance metrics. It is expected that this Clinical Pharmacist I will collaborate with colleagues and provide excellent customer service to patients, staff and serve as the medication expert and main point of contact to provide support for providers, staff and patients, when needed. Additional duties may include quality and performance improvement activities or others as determined by Pharmacy Leadership.As a successful candidate, you will:
  • Collaborate with providers, nurses, pharmacists, and other clinical staff to ensure the medication management process is safe, high quality, and cost-effective for all patients.
  • Verify pharmacy technician's order entry of prior to admission medications into Epic.
  • Perform medication reconciliation of new surgical, endocrine and hematology/oncology patients in the clinic.
  • Compare prior to admission medications with inpatient medications for discrepancies and contact the provider to resolve discrepancies.
  • Help identify medications that may need prior auth or special approvals for discharge and support the coordination around these medications.
  • Perform clinical review of discharge medications for appropriateness and counsel the patient or caregiver upon discharge.
  • Perform post discharge calls to patients within 48-72 hours to follow up on adherence and medication understanding.
  • Provides training, ongoing feedback, and coordinates daily responsibilities of the technicians to ensure adherence to the standard operating procedures.
  • Develops and maintains technician competencies to ensure policies, procedures, and optimal care is always provided.
  • Obtains and maintains licenses and certificates required for job function and completes all assigned trainings in a timely manner.
  • Performs other duties as assigned by the Leadership team.
QualificationsYour qualifications should include:
  • Graduate of ACPE-accredited Pharmacy School or School with ACPE accreditation pending
  • Required Courses/Training: ASHP Medication History-Taking Certificate within 3 months of hire date
  • Current CA Pharmacist License
  • BLS certification through the American Heart Association
  • Two years of hospital experience. Oncology experience, preferred
