- Seeking a qualified professional to deliver physical therapy services to adult patients in their homes, following physician’s directives.
- Conduct patient evaluations using diagnostic tests and develop treatment plans.
- Monitor patient progress, update treatment plans, and communicate with physicians as necessary.
- Document patient visits and maintain up-to-date records.
- Provide specialized treatments to maximize patient functionality and independence.
- Educate patients and caregivers on treatment plans and goals.
- Attend staff meetings and participate in case reviews.
- Ensure safety practices and equipment maintenance.
- Graduate of an accredited Physical Therapy program.
- Valid Physical Therapy license issued by NYS Department of Education.
- Minimum one year of experience as a licensed physical therapist.
- Primarily non-office setting with exposure to outdoor conditions.
