Role and Responsibilities:
  • Seeking a qualified professional to deliver physical therapy services to adult patients in their homes, following physician’s directives.
Essential Functions:
  • Conduct patient evaluations using diagnostic tests and develop treatment plans.
  • Monitor patient progress, update treatment plans, and communicate with physicians as necessary.
  • Document patient visits and maintain up-to-date records.
  • Provide specialized treatments to maximize patient functionality and independence.
  • Educate patients and caregivers on treatment plans and goals.
  • Attend staff meetings and participate in case reviews.
  • Ensure safety practices and equipment maintenance.
Education Requirements:
  • Graduate of an accredited Physical Therapy program.
Qualifications and Requirements:
  • Valid Physical Therapy license issued by NYS Department of Education.
  • Minimum one year of experience as a licensed physical therapist.
Work Environment:
  • Primarily non-office setting with exposure to outdoor conditions.
Physical Requirements:Physically able to stand, sit, hear, walk, stoop, kneel, and perform repetitive motions on a daily basis.
