The Home Health Aide (HHA) is an essential member of the home care team who works under the supervision of a nurse within specified geographical territory. The HHA provides compassionate support with activities of daily living and basic personal care needs while ensuring a safe, comfortable living environment. The HHA effectively communicates and documents any changes in patient conditions to the registered nurse or supervisor and maintains positive relationships. The Company adopts the home health aide scope of practice as outlined by the state for its HHA personnel. The HHA/Bath Aide is responsible for any and all practices and duties within the scope of practice as outlined by the state.WHAT WE OFFER:

Eligible teammates will also receive paid time off, opportunity to participate in 401k, company paid life insurance and access to a robust Employee Assistance Program.QUALIFICATIONS & ATTRIBUTES:High school diploma or equivalent.State Tested Nurse Aide (STNA) or 75-hour nurse aide training (CNA).One year of experience as a nurse aide in a hospital, nursing home, home care agency, or a private setting under the direct supervision of a RN, which can be verified.Completion of skills competency test with 80% or higher score.Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers.Ability to remain calm, have patience and be accommodating. Compassionate and caring while working with patients.Ability to follow directions and work as a team member.Must be observant and detail oriented.Valid driver’s license and auto insurance with your name as a listed driver.MAJOR AREAS OF RESPONSIBILITY:Performs personal care including but not limited to: bathing, grooming, skin care, toileting, and oral hygieneLight housekeeping after completion of personal care, as applicablePreparation of meals and assists with laundry as neededAssists patients as needed with lifting and transferringPerforms simple exercises as an extension of therapy services such as: ambulation and range of motionMaintains good relationships with the patient, the patient’s family, and staffEnsures the living environment is healthy and safeReports any changes to the patient’s medical and living conditions to the supervisor or the case managerCompletes documentation appropriately and timelyUnderstands and practices agency policies and proceduresCommunicates changes in schedule/availability to schedulers or supervisorFollows all legal rules and regulationsMaintains 12 hours of in-services each calendar yearAttends mandatory in-servicesESSENTIAL FUNCTIONS:Drives to patient’s primary location per scheduled visit.Capable of all physical demands of lifting, transferring and bathing a physically dependent individual.Daily attendance at assigned visit locations.Documents all aspects of subsequent care visits within 24 hours of visit. Completes and submits all required documentation within specified company requirements.Specialized Skills: Follows plan of care as permitted within the scope of practice for a home health aide per state of employment.HEALTH QUALIFICATIONS:
