We are recruiting for the following position:Sea Mar is a mandatory COVID-19 and flu vaccine organizationPathways Community Health Worker – Posting #26257Hourly Rate: $21.75Position Summary:Full-Time position available for a Pathways Community Health Worker; will travel throughout Clark County with a base site in Vancouver. This position requires the CHW to be able to drive to meet clients, flexibility, self-initiative, and the ability to manage time, resources, and client caseload efficiently.Under the direction of the Pathways Supervisor, this position works closely with all clients, community organizations, and other departments. This culturally and geographically connected individual serves as a link between underserved communities and existing community resources. Through home visiting, this individual assists clients in overcoming barriers to health, social services, education and employment.This person must have a valid driver license, proof of auto insurance and a vehicle safe for daily use. Prior to hire this person must submit a driver’s abstract demonstrating that they are safe to drive as deemed by Sea Mar’s Health and Safety/Compliance Officer. Within the course of employment, a current driver’s abstract may be requested.Requirements and/or Experience:
  • High School Diploma or Equivalent required.
  • Experience working with people from various backgrounds required.
  • Experience working with individuals with substance use disorders, homelessness, chronic mental illness, or people in crisis preferred.
  • Prefer one year paid or volunteer experience providing medical, social service or behavioral health case management or care coordination.
  • Bilingual English/Spanish required.
