Salary Range: $20.50 to $21.73

Internal Applicants: Apply via email, following the emailed application instructionsPosition Summary: The Housing Case Manager provides ongoing housing case management for clients enrolled in the Eviction Prevention Program in Lane County. The Housing Case Manager works with clients facing eviction or other imminent threat of homelessness in collaboration with the Lane County Human Services Division. The Housing Case Manager works directly with clients to achieve long and short-term housing and housing related stability goals. The role of the Housing Case Manager includes assisting clients in identifying housing stability related barriers and developing client life skills, providing financial assistance, assessments for psychosocial needs, creating and reviewing client budgets and care plans, and linking clients to benefits and resources such as health insurance, social security, community resources, and other medical and social assistance programs. Often, these services are delivered over the phone, in the home or in the community, to reduce clients' barriers to access. In addition, this position coordinates access to support services provided through the HIV Alliance and in the community. The Housing Case Manager utilizes several systems such as Outlook, Cognito Forms, Wellsky Homeless Management Information System, as well as financial assistance software.Essential Functions and Responsibilities:
- Travel: Occasional travel across service area to provide in-person services
- Client Management: Carry an active caseload of program-eligible clients, providing direct one-on-one supportive case management and housing navigation services. Able to provide services in-person, over the phone and on a drop-in basis. Assist with coverage of services.
- Assessments & Planning: Conduct program intakes, comprehensive assessments, and goal planning to identify needs and barriers. Develop individualized service plans based on assessments and available resources.
- Housing Support: Work with the client to identify barriers to housing stability, work with client to identify strategies to overcome these barriers.
- Basic Needs Support: Assist clients in signing up for and accessing health insurance, SNAP, TANF, Social Security and other assistance programs.
- Coordination of Services: Work collaboratively with internal and external partners, landlords, to facilitate access to housing, medical care, social services, mental health services, and public benefits.
- Documentation: Maintain accurate and timely documentation of client interactions, placements, services provided, and progress updates in agency and program databases. Process and collect necessary eligibility documents including financial assistance documentation and Authorization for Release of Information.
- Crisis Intervention: Respond to client crises, including eviction prevention and emergency relocation assistance, and follow up.
- Referrals: Refer clients to local resources as needed and follow up.
- Community Engagement: Maintain good working relationships with Lane County Human Services Division, build and maintain relationships with local property managers, housing authorities, and shelters to enhance housing placement opportunities. Attend relevant community meetings and participate in ongoing training and professional development.
- Client Events: Assist with covering services and events in other teams on occasion
- Standards: Comply with all applicable program standards, guidelines, laws and administrative rules.
- Other tasks as assigned, including training/cross-training other staff as needed
- Bachelor's degree in social work/human services related field or 2 years full-time case management experience, or equivalent combination of relevant and/or related experience and education
- TB test (provided at agency)
- Valid Oregon driver's license, or valid out of state driver's license with the ability to get an Oregon driver's license within 30 days of establishing residency in Oregon, and a driving record sufficient to be covered by agency auto insurance policy.
- Ability to pass agency criminal background check
- Ability to obtain OHA Application Assister ID (cost covered by agency)
- Ability to obtain Food Handler's Card (cost covered by agency)

- Sit and/or stand for extended periods of time at an assigned station
- Bend, stand, & reach to be able to transport clients and to lift and carry up to 25 pounds for tasks such as building kits & assembling testing/educational materials.
- Use continuous clarity of vision at 20 inches or less for processing of paperwork
- Ability to work flexible hours, including early mornings, evenings, and weekends
- Ability to work 5 days per week for a minimum of 20 hours per week
- Ability to work in loud, crowded, busy, or escalated environments
- Ability to drive at night and during various weather conditions safely
