Job Overview – Nurse Manager (Home Health Care):
Compensation: $140,000 – $150,000/year
Location: Stamford, CT
Schedule: Monday to Friday, 9:00 AM – 5:00 PM (In-Office with Local Travel)Our client is seeking a Nurse Manager (Home Health Care) in Stamford, CT, to lead clinical operations, nursing staff management, and patient care. You will oversee field nurses, manage admissions and care planning, ensure compliance with OASIS and CMS regulations, and conduct quality reviews. Ideal candidates are experienced RNs with leadership skills and expertise in home health standards.Responsibilities as the Nurse Manager (Home Health Care):
  • Clinical Supervision: Lead, mentor, and evaluate field nurses and clinical staff through education, performance reviews, and ongoing support.
  • Care Coordination: Oversee patient admissions, care planning, and case management to ensure timely, high-quality care.
  • Regulatory Compliance: Ensure accurate and timely OASIS documentation, Plans of Care, and 485s in alignment with CMS and state requirements.
  • Quality Assurance: Conduct chart reviews and home visits to maintain clinical excellence and compliance standards.
  • Collaboration: Partner with physicians, caregivers, and families to develop and adjust individualized care plans.
  • Staffing & Recruitment: Support hiring, onboarding, and retention of qualified nursing staff to strengthen the home health team.
Qualifications for the Nurse Manager (Home Health Care):
  • Licensure: Active and unrestricted Registered Nurse (RN) license in Connecticut required.
  • Education: Bachelor of Science in Nursing (BSN) strongly preferred.
  • Experience: 5+ years of nursing experience, including 3+ years in home health and 2+ years in a supervisory or managerial role.
  • Technical Skills: Proficiency in Home Care Home Base (HCHB) or similar EMR systems, with strong Microsoft Office skills.
  • Industry Knowledge: In-depth understanding of OASIS documentation, CMS Conditions of Participation, and state home health survey requirements.
  • Skills & Attributes: Strong leadership, organizational, and communication abilities, with excellent critical thinking and problem-solving skills.
  • Additional Requirements: Valid driver’s license and reliable transportation for local travel to patient homes.
