  • Deliver routine nursing care with empathy and precision
  • Champion chronic care management for high-risk patients
  • Coordinate care for elderly and disabled individuals with conditions like diabetes, HIV, hypertension, and obesity
  • Travel between Stamford and Greenwich to support patients where they are
  • Assist with referral coordination and specialist support
  • Collaborate with a passionate, multidisciplinary team
🧠 What You Bring
  • Active LPN license in Connecticut
  • 2+ years of hands-on nursing experience
  • Background in primary care or outpatient settings
  • Expertise in chronic care and population health
  • EHR proficiency and strong documentation skills
  • Bilingual in English and Spanish? That’s a big plus!
