- conducts individualized in-home assessments and support planning that assists in the identification and authorization of cost-effective, outcome-oriented service options that address individuals' needs;
- works with interdisciplinary team members to ensure effective case coordination;
- provides information about publicly funded long-term supports and services and assists participants throughout the eligibility process;
- assists participants and family members in obtaining service options and maintenance of basic needs such as housing, food, medical care, and medication ;
- maintains accurate and up-to-date participant records;
- provides information and/or referral upon request by telephone, mail, email, walk-ins, outreach, health fairs, and/or group presentations;
- complies with Federal, State, and Local laws, rules, regulations, and best practices in direct service delivery;
- participates in on-going training and cross-training events for community service providers;
- provides input regarding the development and regular review of the Division marketing plan;
- performs other duties as assigned.
Employees appointed to temporary positions may also gain membership in the civil service upon satisfactory completion of an initial probation period of at least six months.
