Duration: 13 weeks
Shift: Day shift, 5x8 hours (08:00 AM – 04:30 PM)Job Responsibilities:
- Case management and utilization review including admission criteria, discharge planning, appeals/denials, concurrent and retrospective reviews.
- Conduct biopsychosocial assessments and develop care coordination plans.
- Utilize InterQual and Milliman guidelines to determine medical necessity.
- Coordinate services across home care, rehab, and long-term care referrals.
- Documentation of care delivery and compliance with regulatory standards.
- Provide care for patients across multiple units including Med Surg (required), Emergency Department, ICU, NICU, and L&D/Mother-Baby (preferred).
- Possible floating to other Swedish hospitals.
- Support patient populations including infants, adults, and geriatrics with daily census of 18-20 patients.
- Active Washington State LMSW license or associate clinical license/LCISW.
- Minimum 2 years of experience in case management and discharge planning.
- Knowledge of CMS, CPT, Dept of Health, DRG, HEDIS, HIPAA, ICD 10, NCQA, OSHA, Joint Commission standards.
- Proficient in Epic charting system.
- BCLS certification.
- Experience with Medicaid, Medicare, and community-based programs.
- Ability to float within hospital system.
- Strong knowledge of medical social work in acute care settings, especially Med Surg.
- Utilization management experience.
- Experience with trauma, emergency department, ICU, NICU, and L&D/mother-baby units.
- Familiarity with APS, cultural competency, high-risk interventions, NASW code of ethics.
- Additional certifications and training related to social work preferred but not mandatory.
- Local candidates at travel rates are considered.
- Must provide proof of current WA license and local residence (within 50 miles preferred).
