In this role, you will be responsible for taking vital signs and monitoring health data for eligible patients at various long-term care and assisted living facilities. You will play a critical role in ensuring that accurate health information is collected and communicated, supporting our remote patient monitoring efforts.Key Responsibilities:● Vital Signs Monitoring: Conduct daily assessments of patients' vital signs including blood pressure, heart rate, respiratory rate, and temperature in accordance with established protocols.● Data Collection and Reporting: Accurately record and report vital signs data using our electronic health record (EHR) system or other designated tools. Ensure timely submission of data to support remote monitoring and care coordination.● Patient Interaction: Establish a compassionate and professional rapport with patients to ensure their comfort and cooperation with remote patient monitoring program during assessments.● Facility Collaboration: Work closely with facility staff and healthcare providers to coordinate patient monitoring schedules and address any concerns or issues related to vital signs monitoring.● Compliance and Documentation: Adhere to all relevant healthcare regulations and guidelines to maintain patient privacy and data security.
